healthAbility's Reconnect Counsellor & Case Manager supports young people aged 12-18 who are at risk of homelessness, using a strengths-based, holistic approach to prevent homelessness and improve wellbeing. The role sits in the Children, Youth and Family team, co-located at the Nillumbik Youth Hub with a strong Outreach focus.

The position is part-time (0.6 FTE) until 30 June 2027, with responsibilities that include caseload management of 20-30 young people per year and collaboration with
