About Us

The Acute Neuropsychology Unit is situated within Monash Medical Centre, Clayton. We provide inpatient consultation across all Monash Health acute sites for adult medical, oncology, cardiac and surgical services, alongside a specialist Outpatient Neuropsychology Clinic for current Monash Health patients.

Our team works with people experiencing neurological, neurodegenerative and complex medical conditions affecting cognitive functioning. We collaborate closely with multidisciplinary teams and play a key role in supporting patient care.

About the Role

This is an exciting opportunity to join a highly specialised neuropsychology service providing assessment and consultation across inpatient and outpatient settings.

Key responsibilities include:

• Conducting neuropsychological, cognitive and decision-making capacity assessments.
• Providing consultation, recommendations and comprehensive reports to treating teams.
• Contributing to multidisciplinary care planning and service development.
• Supporting and supervising students and Grade 2 psychologists.

About You

You are an experienced Clinical Neuropsychologist who values collaborative practice and has expertise in assessing and managing adults with neurological and medical conditions.

You will have:

• General registration as a Psychologist with AHPRA and endorsement in Clinical Neuropsychology.
• At least five years' post-registration experience as a Grade 2 Psychologist (or equivalent).
• Expertise in neuropsychological assessment of complex neurological and medical presentations.
• Strong clinical reasoning, communication and report-writing skills.
• Experience providing consultation, supervision and professional support to others.
• A commitment to evidence-based, patient-centred care.
• You are a board approved supervisor with the Psychology Board of Australia or would be eligible to undergo the appropriate training within 6 months of commencing the role
• You are a collaborative team player who values partnership, shared expertise and working closely within a multidisciplinary team

About the role

• Provide an integrated, patient-centred and evidence-based Occupational Therapy service to inpatients across the St Vincent’s Hospital campus, including managing new referrals, reviewing patients and continuing therapy as outlined in handover lists. Support other clinical areas as required to meet hospital-wide demand and contribute to a seven-day rehabilitation service, including weekend shifts

• Determine patients’ occupational needs through standardised and/or functional assessments, and design and implement patient-centred interventions with a focus on therapy, discharge planning and patient flow

• Demonstrate effective time management and organisational skills to prioritise and manage clinical and non-clinical tasks within finite resources, ensuring optimum standards of care

• Work effectively both independently and within a multidisciplinary team to provide coordinated, patient-centred care, with the ability to independently manage an allocated caseload and responsibilities when working without direct team support

• Demonstrate responsible use of Occupational Therapy equipment, Department and organisational resources

• Participate in clinical supervision, as well as contribute to Occupational Therapy service evaluation, quality improvement and research activities as indicated

About you

• Recognised Degree/Diploma in Occupational Therapy, current Australian Occupational Therapy Registration (AHPRA), and a current unrestricted NSW Class C Driver’s Licence

• Previous post-graduate Occupational Therapy experience in acute and sub-acute hospital settings across a range of clinical areas, including neurology, general medicine/surgery and rehabilitation, within a multidisciplinary team environment

• Demonstrated experience in clinical management , with the ability to apply sound clinical reasoning and independent professional judgement in routine matters

• Demonstrated strong communication, teamwork and interpersonal skills, with the ability to work effectively within a multidisciplinary team

About the Role

• Lead the Quality Use of Medicines (QUM) program across SVHS and affiliated services, providing clinical and professional leadership to support safe and sustainable medication practices, compliance with medication management standards, and alignment with SVHA and NSW Health priorities

• Oversee the hospital medication formulary, including Individual Patient Use (IPU) approvals, formulary requests and reporting, and liaison with the State Formulary Committee and that all regulatory and legislative requirements are met.

• Deliver Medication Use Evaluations and Quality Improvement activities, including monitoring and reporting on drug expenditure, and contribute to QUM research, education, promotional initiatives, bulletins and memos supporting safe and effective medication use

About you

• Registered pharmacist with AHPRA, with no conditions on scope of practice, and relevant post-registration clinical or management/leadership qualifications or equivalent skills and experience. Availability for on-call and weekend work as required

• Significant experience and demonstrated competence in clinical pharmacy services, with strong clinical judgement, expertise and commitment to ongoing professional development

• Demonstrated experience in formulary management and Drug and Therapeutics Committee processes, including implementation of committee decisions and development and implementation of medication-related policies and procedures

• Demonstrated experience delivering Quality Use of Medicines (QUM) initiatives and Medicines Use Evaluation (MUE/DUE) activities, with knowledge of applying relevant Healthcare Standards in practice

About the Team

The Community Mental Health Continuing Care Team and Mobile Support and Treatment Services works across Clayton, Southern and Bayside regions to provide an effective and comprehensive service to consumers of the Clayton Adult Mental Health Program. Working within a multi-disciplinary team we provide treatment and support to individuals experiencing complex mental health and psychosocial challenges.  

The Continuing Care Team Model of Care provide follow up to consumers with mental health issues requiring specialist mental health treatment and support that often is not met by generic services. This service provides case management support and treatment for consumers aged 25 to 65 years, operating Monday to Friday business hours, based at Atherton Rd Centre, Oakleigh.

The Mobile Support and Treatment Services is an assertive outreach services which provides case management to consumers aged between 25-65 years of age, with complex mental health needs who require psychosocial support and treatment. This service operates Monday – Sunday, working a rotating roster of morning and afternoon shifts, based at Atherton Rd Centre, Oakleigh.

About the Role 

An exciting opportunity is available for a Grade 2 Physiotherapist to join the Continence Service within Specialist Clinics. Based at Monash Health's Thomas Street, Dandenong and Kingston Centre, this role also offers the opportunity to travel to other Monash Health sites as required.

In this role you will play a key role in delivering best practice, patient-centred care while working alongside Senior Clinician Physiotherapists and Stream/Team Leaders. This role offers the chance to:

• Provide specialist continence physiotherapy assessment, management, and treatment for clients through the Monash Health Community Continence Service
• Manage your own independent caseload with a focus on pelvic floor, bladder, and bowel dysfunction
• Monitor and evaluate client outcomes to ensure care meets both best practice standards and community expectations
• Develop, implement, and educate clients in individualised pelvic floor rehabilitation programs
• Collaborate across services to build strong internal and external partnerships

About the Role

We’re looking for an experienced and compassionate Grade 2 Social Worker to join our Community Rehabilitation team in a part-time, ongoing position, predominantly based at Kingston Centre in Heatherton.

Community rehabilitation is about more than supporting someone through an episode of care. It’s about understanding the person behind the diagnosis – their home, family, relationships, responsibilities and the things that matter most to them – and helping them move forward with confidence and the right supports around them.

Working within an experienced interdisciplinary team, you’ll provide short-term, goal-focused Social Work intervention to adults across a diverse range of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation.

Your work will include:

• Providing psychosocial assessment, intervention, case management and discharge planning
• Supporting clients and their families or carers to navigate change, adjustment and complex circumstances
• Working with clients where family violence may be suspected or disclosed
• Connecting people with community services, supports and funding pathways
• Collaborating closely with the broader interdisciplinary team to achieve meaningful client goals
• Liaising with hospital and community-based services to support continuity of care
• Providing clinical supervision to Grade 1 Social Workers and Allied Health Assistants, as allocated
• Supporting the learning and development of Social Work students
• Contributing your ideas and experience to service development, evidence-based practice and quality improvement

About Community Rehabilitation

Monash Health Community Rehabilitation helps people improve and maintain their health, independence and wellbeing following illness, injury or a change in their health. Our care is delivered where it works best for the client – at home, at one of our Community Rehabilitation centres or through tele-rehabilitation – and may include individual or group-based intervention.

Our approach is time-limited, goal-focused and tailored to the individual. We look beyond a person's diagnosis to understand what they want to get back to doing and what might be standing in their way. As a Social Worker, you’ll bring an important perspective to that journey, helping clients and families navigate the social, emotional and practical impacts of changes to their health while connecting them with the supports they need to continue living well in their community.
