About the role
Lead the Quality Use of Medicines (QUM) program across SVHS and affiliated services, providing clinical and professional leadership to support safe and sustainable medication practices, compliance with medication management standards, and alignment with SVHA and NSW Health priorities
Oversee the hospital medication formulary, including Individual Patient Use (IPU) approvals, formulary requests and reporting, and liaison with the State Formulary Committee and that all regulatory and legislative requirements are met.
Deliver Medication Use Evaluations and Quality Improvement activities, including monitoring and reporting on drug expenditure, and contribute to QUM research, education, promotional initiatives, bulletins and memos supporting safe and effective medication use
Coordinate the SVHN Drug and Therapeutics Committee (DTC) secretariat, including communicating DTC decisions, preparing reports and maintaining the Pharmacy Intranet, while providing QUM advice to clinicians, pharmacists and external organisations including the Ministry of Health and CEC
Be an active member of relevant committees and working parties and assist in the development and implementation of organisational policies and procedures to ensure optimal medication management
About you
Registered pharmacist with AHPRA, with no conditions on scope of practice, and relevant post-registration clinical or management/leadership qualifications or equivalent skills and experience. Availability for on-call and weekend work as required
Significant experience and demonstrated competence in clinical pharmacy services, with strong clinical judgement, expertise and commitment to ongoing professional development
Demonstrated experience in formulary management and Drug and Therapeutics Committee processes, including implementation of committee decisions and development and implementation of medication-related policies and procedures
Demonstrated experience delivering Quality Use of Medicines (QUM) initiatives and Medicines Use Evaluation (MUE/DUE) activities, with knowledge of applying relevant Healthcare Standards in practice
Excellent written, verbal and interpersonal communication skills, with the ability to develop high-quality written materials, communicate effectively, negotiate and influence stakeholders, and contribute to a positive workplace culture
Strong problem-solving, organisational and time-management skills, with the ability to manage competing priorities, work autonomously and deliver complex tasks with accuracy, attention to detail and accountability

About the Role
Under the direction of the Manager and Team Consultant Psychiatrist and in collaboration with team members, develop and implement strategies that support tenants of public housing requiring mental health and drug and alcohol support and intervention to engage with STAY, to connect with community, referral and other health supports. You will provide peer support to consumers who are part of the program, as well as offering a lived experience perspective to the clinical staff and housing services that consult with us.
You have the necessary skills to work effectively within a multi-disciplinary team, build and maintain strong professional relationships with internal and external health professionals. You feel comfortable working in a ‘non-traditional’ mental health role and are open to new learning. You will contribute to the development and delivery of tenants’ support and referral plans through effective engagement and connection
You are highly motivated, innovative and enthusiastic and can apply your lived experience and peer support skills to the support and recovery of others.
- Work within the Strengths Model of recovery orientated practice
- Build a therapeutic alliance with the consumer and family/carers and actively engage them in peer support
- Work within a Community Development and consultation framework, advising from a lived experience perspective.
Classifications for this position will range from Lived Experience Worker Level 4 AK47 – AK50 ($93,605 - $104,270 per annum pro rata)

About Mental Health
St. Vincent’s Mental Health (SVMH) includes an adult area mental health service for the inner city areas of Yarra and Boroondara.
The STAY program provides ongoing specialist treatment, support, and consultation to meet the identified needs of people who have potential mental health presentations or concerns and are tenants in local public housing properties in the City of Yarra. As a Gambling Help Community Educator, you'll play a vital role working with clients and the community to provide education and information, focusing on empowerment and resilience outcomes for the client, family and community wellbeing. You'll work across communities to deliver engaging training and awareness programs that promote safer gambling practices and strengthen wellbeing outcomes.
Key Responsibilities:
In this varied and community-based role, you will:
- Design and deliver community education and awareness programs focused on safe gambling and harm minimisation.
- Facilitate engaging workshops, presentations and training sessions for a diverse audience.
- Build strong relationships with community organisations, services and referral networks.
- Promote early intervention and improve awareness of gambling-related harm
- Collect and maintain accurate program data, outcomes and case studies.
- Research emerging trends and contribute to continuous improvement initiatives.
- Collaborate with internal and external stakeholders to improve community outcomes.

Provide evidence-based psychological assessment, formulation and intervention for patients with osteoarthritis of the knee or hip, supporting pain management, health behaviour change, adjustment to long-term conditions, function and quality of life

About the Role
Join the Casey Adult Crisis Intervention Service (ACIS) as a Grade 4 Mental Health Social Worker and play a key role in delivering timely, recovery-oriented care to consumers experiencing an acute mental health crisis. This is a full time, ongoing position, based in Cranbourne.
Working across the community, clinic and via telehealth, you will undertake comprehensive mental health, psychosocial and risk assessments, provide crisis intervention, and develop collaborative treatment and safety plans. You will work closely with consumers, families, carers and a broad multidisciplinary network to support people to remain safely in the community wherever possible.
This is a senior clinical role where you will apply your expertise in mental health legislation, risk assessment and crisis management while partnering with inpatient units, emergency departments and community agencies to ensure seamless care and positive consumer outcomes. The position operates across a seven-day rotating roster including AM and PM shifts.

The Casey Adult Crisis Intervention Service (ACIS) forms part of the Casey Area Mental Health Service, supporting one of Melbourne's fastest-growing communities. We provide intensive, short-term assessment and treatment for adults experiencing an acute mental health crisis, delivering care in consumers' homes, within our clinic and via telehealth.

- Actively provide support to the strategic Hospitality plan.
- In conjunction with the Resident Experience Manager, ensure that all menus have been nutritionally assessed and menu reviews involve and include resident involvement.
- Recipes are trialled and tested using the available resources at the sites, and production staff, along with the target audiences, are actively engaged in the process.
- In conjunction with the Resident Experience Manager, participate in the development and monitoring of recipes in line with standard production practices. This will include the use of software where applicable and also require the monitoring of nutritionally balanced meal production.
- Provide support and technical skills to CPU based personnel in relation to Cook Chill processes and systems, including recruitment, training and ongoing professional development of staff.
- Provide support and technical skills to site based personal in relation to Cook Chill processes and systems including product regeneration and quality.
- Effectively communicate with all stakeholders keeping in mind "cause and effect" of changes implemented by you/ team.
- Effectively manage food safety plan compliance for the Bundaberg CPU.
- Report to the Resident Experience Manager on non-compliances and incidents and action taken to rectify.
- Report to the Resident Experience Manager on Bundaberg CPU financial performance KPI’s.
- Control stock management procedures in line with best industry practices
- Support the procurement function to maximise the returns for the organisation.

- Ensure that the work carried out is in keeping with the Blue Care Food Safety Manual and safe practices as per relevant WH&S legislation.
- Assist with and also drive the implementation of Quality Feedback Tools to capture and review end user satisfaction levels in consultation with the direct report and organisational resources.
Essential Qualifications & Licences:
- Qualified Chef at Senior Sous Chef, Executive Chef, or equivalent level
- A relevant trade or operational qualification (e.g. Certificate III Commercial Cookery or similar), or equivalent supervisory experience
- Minimum 3-5 years' experience within large-scale food service operations
- Demonstrated experience managing commercial or institutional food production environments
- Current Open Driver Licence
- A sound knowledge of Cook Chill Operations – preferably within Institutional Facilities.

Join our team as a Clinical Educator Pharmacist on a part time or full time, ongoing basis, working across all our Monash Health sites. In this role, you will be guided under the leadership of the Pharmacist Team Leader for Clinical Educators, working with the broader pharmacy and multidisciplinary teams.
Key responsibilities include:

About the Role
We’re looking for an experienced and compassionate Grade 2 Social Worker to join our Community Rehabilitation team in a part-time, ongoing position, predominantly based at Kingston Centre in Heatherton.
Community rehabilitation is about more than supporting someone through an episode of care. It’s about understanding the person behind the diagnosis – their home, family, relationships, responsibilities and the things that matter most to them – and helping them move forward with confidence and the right supports around them.
Working within an experienced interdisciplinary team, you’ll provide short-term, goal-focused Social Work intervention to adults across a diverse range of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation.
Your work will include:
- Providing psychosocial assessment, intervention, case management and discharge planning
- Supporting clients and their families or carers to navigate change, adjustment and complex circumstances
- Working with clients where family violence may be suspected or disclosed
- Connecting people with community services, supports and funding pathways
- Collaborating closely with the broader interdisciplinary team to achieve meaningful client goals
- Liaising with hospital and community-based services to support continuity of care

Monash Health Community Rehabilitation helps people improve and maintain their health, independence and wellbeing following illness, injury or a change in their health. About the Role
An exciting opportunity is available for a Grade 2 Physiotherapist to join the Continence Service within Specialist Clinics. Based at Monash Health's Thomas Street, Dandenong and Kingston Centre, this role also offers the opportunity to travel to other Monash Health sites as required.

About the Role
Join our large, supportive and well-established Occupational Therapy team within Country Health Connect, delivering high-quality services across the Limestone Coast.
We are seeking both experienced Occupational Therapists and new graduates to join our candidate pool. Opportunities may arise across a broad range of clinical areas including acute care, subacute, aged care, paediatrics, rehabilitation, disability, outreach and community services. Working within a multidisciplinary team, you will provide evidence-based assessment, intervention, rehabilitation and health promotion services that support clients to achieve greater independence, improve functional outcomes and enhance quality of life. With opportunities to rotate across clinical settings, receive structured clinical supervision and develop specialist interests, this role offers an excellent foundation for career progression in rural generalist practice.
About You
You are a passionate Occupational Therapist who enjoys working collaboratively to deliver person-centred care across diverse clinical settings. Whether you are an experienced clinician or a new graduate looking to build your career, you are committed to continuous learning, professional development and improving health outcomes for regional communities.
To be successful, you will have:
- A recognised qualification in Occupational Therapy and eligibility for registration with the Occupational Therapy Board of Australia (AHPRA).
- Excellent communication and interpersonal skills with the ability to work effectively within multidisciplinary teams.
- A commitment to delivering high-quality, evidence-based and client-centred care.
- A current South Australian driver's licence and willingness to travel throughout the Limestone Coast region.
- Experience in a range of clinical settings is desirable for AHP2 positions, while new graduates are encouraged to apply for AHP1 opportunities.

Appointment will be subject to:
- A current Nationally Coordinated Criminal History Check (NCCHC).
- DHS Working With Children Check (WWCC).
