We provide safe and equitable care, for every stage of life, delivered with world-leading expertise and over 175 years of experience. As a leading academic health service, we're committed to providing tomorrow's care today by integrating research, teaching and training into everyday practice.
Our team of 24,000 people are full hearted, open minded and all in. Together, we provide more than 250 integrated services for every stage of life at over 50 locations and in the community across south-east Melbourne, Victoria, serving 1.2 million residents in the South Metro Local Health Service Network.
To see first-hand what our colleagues think about working here, take a look at the following short videos monashhealth.org/careers.
About the Role
Join the Casey Adult Crisis Intervention Service (ACIS) as a Grade 4 Mental Health Social Worker and play a key role in delivering timely, recovery-oriented care to consumers experiencing an acute mental health crisis. This is a full time, ongoing position, based in Cranbourne.
Working across the community, clinic and via telehealth, you will undertake comprehensive mental health, psychosocial and risk assessments, provide crisis intervention, and develop collaborative treatment and safety plans. You will work closely with consumers, families, carers and a broad multidisciplinary network to support people to remain safely in the community wherever possible.
This is a senior clinical role where you will apply your expertise in mental health legislation, risk assessment and crisis management while partnering with inpatient units, emergency departments and community agencies to ensure seamless care and positive consumer outcomes. The position operates across a seven-day rotating roster including AM and PM shifts.
About the Team
The Casey Adult Crisis Intervention Service (ACIS) forms part of the Casey Area Mental Health Service, supporting one of Melbourne's fastest-growing communities. We provide intensive, short-term assessment and treatment for adults experiencing an acute mental health crisis, delivering care in consumers' homes, within our clinic and via telehealth.
Our team is committed to recovery-oriented, person-centred care and works collaboratively with consumers, carers, families and a wide network of health and community partners. Supported by experienced and approachable leaders, you'll join a highly skilled multidisciplinary team that values clinical excellence, innovation and continuous learning.
About You
You are an experienced Mental Health Social Worker with a passion for delivering high-quality, recovery-focused care in fast-paced environments. Confident working with complexity, you bring advanced assessment, crisis intervention and risk management skills, along with a sound understanding of the Mental Health and Wellbeing Act and contemporary mental health practice.
You are a collaborative clinician who builds strong relationships with consumers, families, carers and multidisciplinary teams, and you thrive in a role where no two days are the same. Your excellent communication skills, sound clinical judgement and commitment to person-centred care enable you to confidently navigate complex presentations while supporting safe and effective outcomes.
To succeed in this role, you will also bring:
- Eligibility for membership with the Australian Association of Social Workers (AASW)
- Extensive post-qualification experience in mental health, including crisis assessment and intervention
- Demonstrated ability to work collaboratively across inpatient, emergency and community settings
- A commitment to evidence-based practice, quality improvement and ongoing professional development
Why Join Us?
This is an opportunity to bring your experience to a senior clinical role where your expertise, judgement and leadership can make a genuine difference.
At Casey ACIS, you’ll be there when your clinical skills matter most — supporting consumers and families through some of their most challenging moments and helping people access the right care while remaining safely connected to their community wherever possible.
You’ll join an experienced and supportive multidisciplinary team where your ideas and expertise are valued. As a Grade 4 Social Worker, you’ll also have the opportunity to influence clinical practice, contribute to service improvement and support the development of others, while continuing to grow your own specialist mental health and leadership capability.
In this role, you will:
- Make a meaningful impact for consumers and families experiencing acute mental health crisis
- Step into a senior clinical role where your expertise and professional judgement are valued
- Work alongside an experienced, collaborative multidisciplinary team
- Help shape clinical practice through leadership, mentoring and service improvement

Help people regain independence, reconnect with what matters to them and live well in their community.
About the Role
We’re looking for an experienced and compassionate Grade 2 Social Worker to join our Community Rehabilitation team in a part-time, ongoing position, predominantly based at Kingston Centre in Heatherton.
Community rehabilitation is about more than supporting someone through an episode of care. It’s about understanding the person behind the diagnosis – their home, family, relationships, responsibilities and the things that matter most to them – and helping them move forward with confidence and the right supports around them.
Working within an experienced interdisciplinary team, you’ll provide short-term, goal-focused Social Work intervention to adults across a diverse range of clinical streams, including neurological, musculoskeletal, general, cardiac and pulmonary rehabilitation.
Your work will include:
- Providing psychosocial assessment, intervention, case management and discharge planning
- Supporting clients and their families or carers to navigate change, adjustment and complex circumstances
- Working with clients where family violence may be suspected or disclosed
- Connecting people with community services, supports and funding pathways
- Collaborating closely with the broader interdisciplinary team to achieve meaningful client goals
- Liaising with hospital and community-based services to support continuity of care
- Providing clinical supervision to Grade 1 Social Workers and Allied Health Assistants, as allocated
- Supporting the learning and development of Social Work students
- Contributing your ideas and experience to service development, evidence-based practice and quality improvement
About Community Rehabilitation
Monash Health Community Rehabilitation helps people improve and maintain their health, independence and wellbeing following illness, injury or a change in their health. Our care is delivered where it works best for the client – at home, at one of our Community Rehabilitation centres or through tele-rehabilitation – and may include individual or group-based intervention.
Our approach is time-limited, goal-focused and tailored to the individual. We look beyond a person's diagnosis to understand what they want to get back to doing and what might be standing in their way. As a Social Worker, you’ll bring an important perspective to that journey, helping clients and families navigate the social, emotional and practical impacts of changes to their health while connecting them with the supports they need to continue living well in their community.
About You
You’re a Social Worker who enjoys variety, collaboration and seeing the real-world impact of your work. You’ll bring:
- Experience providing high-quality, person-centred Social Work assessment and intervention
- Strong clinical reasoning and the ability to confidently manage a varied caseload
- An understanding of trauma-informed practice and working with vulnerable populations
- An awareness of the social, cultural, legal and systemic factors that can influence a person's health and wellbeing
- Strong organisation, prioritisation and time-management skills
- A collaborative approach and willingness to contribute as part of an interdisciplinary team
- The ability to reflect on your practice and continue developing your clinical skills
- A genuine commitment to helping people maintain independence and participate meaningfully in their lives and communities
Qualification & licence requirements are as per the Supportive Documentation below.
Why You’ll Love Working With Us
In Community Rehabilitation, you have the opportunity to work alongside people as they rebuild confidence, independence and connection following changes to their health. You’ll see the difference that good Social Work can make not just clinically, but in someone’s everyday life. You’ll also join a team where you can continue growing your own career, with:
- Support from an experienced Senior Clinician Social Worker and Team Leader, giving you the opportunity to continue developing your clinical practice while also building your skills in supervision, education and service improvement.
- A collaborative interdisciplinary environment where your Social Work perspective is valued
- Exposure to a broad and varied clinical caseload
- Opportunities to develop skills in clinical supervision and student education
- Professional development, mentoring and learning opportunities
- The opportunity to contribute to service improvement and evidence-based practice
- Salary packaging and access to employee wellbeing initiatives
- Fitness Passport and onsite gym options
If you’re looking for a Social Work role where you can build your clinical expertise, enjoy genuine variety and help people get back to living the lives that matter to them, we’d love to hear from you.
For a confidential discussion and to explore this opportunity further, please reach out to Gabrielle Writer, Team Leader, on 0418 973 254.
